Our Commitment to Your Privacy Broadway Family Clinic is dedicated to maintaining the privacy of your Protected Health Information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you.

How We May Use and Disclose Your PHI

  • For Treatment: We may use your PHI to provide medical treatment or services. For example, we may disclose medical information about you to specialized technicians, or other personnel who are involved in taking care of you.

  • For Payment: We may use and disclose PHI so that the treatment and services you receive may be billed to and payment may be collected from you, an insurance company, or a third party.

  • For Healthcare Operations: We may use and disclose PHI for clinic operations, such as quality assessment, employee reviews, and training medical students.

  • Appointment Reminders: We may use and disclose PHI to contact you as a reminder that you have an appointment.

Your Rights Regarding Your PHI

  • Right to Inspect and Copy: You have the right to inspect and copy your medical records.

  • Right to Amend: If you feel that medical information we have about you is incorrect or incomplete, you may ask us to amend the information.

  • Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, or operations.

  • Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose about you.
