  • Many plans cover preventive services at no cost when specific guidelines are met. However, addressing additional concerns during the same visit may result in charges applied to your insurance.

  • In most cases, insurance does not cover 100% of medical costs. Many plans require patients to share in the cost of care through copays, deductibles, and coinsurance, even when services are medically necessary.

  • A deductible is the amount you must pay out of pocket each year before your insurance begins covering certain services. Until your deductible is met, you may be responsible for most or all of the visit cost.

  • A copay is a fixed dollar amount you pay at the time of your visit. Copays vary depending on your insurance plan and the type of visit.

  • Patient responsibilities such as copays, deductibles, and known balances are due at the time of service. Remaining balances are billed after insurance processing.

  • Coinsurance is a percentage of the visit cost that you pay after meeting your deductible. For example, if your plan has 20% coinsurance, insurance covers 80% and you are responsible for the remaining 20%.

  • Out-of-pocket costs are the portion of your healthcare expenses that you are responsible for paying, including deductibles, copays, and coinsurance.

    Additional Clinic Policies

    Appointment Policy

    We kindly request at least 48 hours' notice for appointment cancellations or rescheduling.

    Cancellations made with less than 48 hours’ notice may result in a $25 fee added to your account.

    This fee may be collected at your next visit.

    In emergencies, please notify us as soon as you are aware via email; documentation may be requested.

    No-Show Policy

    Failure to attend a scheduled appointment without prior notice will result in a $25 no-show fee.

    Late Cancellation Policy

    Cancellations made on the same day of your appointment (except emergencies) may incur a late-cancellation fee of $25, as this prevents the slot from being offered to other patients.

    Payment Terms

    Payment is due at the time services are rendered unless prior arrangements are made.

    We accept major insurance plans, credit/debit cards, HSA/FSA cards, and self-pay payments.

    Balances not paid promptly may be sent to collections in accordance with clinic policy.

    Insurance Disclaimer

    As a courtesy, Broadway Family Clinic bills your insurance, but coverage is not guaranteed.

    Patients are responsible for understanding their individual plan benefits, including:

    Please allow 48–72 business hours for routine refills.

    Controlled substances are strictly monitored, require regular follow-ups, and may not be refilled early.

    Our providers do not manage chronic pain regimens or long-term sleep-medication therapy.

    Lost or stolen prescriptions may require documentation and may not be replaced.

    Refill requests made after hours or on weekends will be processed on the next business day.

    Communication Guidelines

    Email is the preferred and most reliable method of non-urgent communication.

    For emergency medical issues, call 911 or visit the nearest emergency room.

    Messages sent through the website may not be monitored continuously; please allow reasonable time for responses.
